How to get physically handicapped certificate for applying gate exam? ? I want physically handicapped certificate please help me out.

Physically handicapped certificate is issued to persons who are orthopaedically handicapped, mentally handicapped or are blind or deaf or dumb.

DOCUMENTS REQUIRED :

1. Fill the application form and get it duly attested by class-1 gazetted officer.

2. Attested photocopy of ration card for residence proof.

3. Attested photocopy of medical certificate in respect of disability from All India institute of medical sciences.

4. Two passport size photographs. one should be attested by gazetted officer. For the temporary ID card Rs.10/- to be deposited to the cashier of the district and attach the receipt to the application form.

5. While submitting the application form original ration card should be showed.

6. ID card will be issued only after getting signature on the card.


* You can submit the application form to the Sub-divisional magistrate of your area on any working day.

* Physically handicapped certificate will be issued with in 21 days.
