The qualifying Examination for appearing at MHT-CET-2010 is HSC (12th Std.) or equivalent examination and conditions for eligibility is defined in the respective brochure i.e Health Sciences, Engineering and Pharmacy degree courses. For student who passed their qualifying examination before 2009(since you passed in 2008), so you can get your application form from your respective Jr. College / School (from where you have passed ).
