Is it possible to get transfer to another college for Diploma Engineering 1st year student?

Hi Sir, I have studied 1st year of Diploma Engineering course in a Government college there I got 3 backlogs. I don't want to study in that college as my health condition is very bad. I want to get transfer to my hometown. Kindly tell me if it is possible or not?
