The JIPMER forms are available in State bank of India(SBI Bank) in the campus of Jipmer
There is another way of getting JIPMER forms. You can get it by post too. Just send a request to the address given in the notification followed by money order. You will soon recieve it You can even apply online at www.jipmer.edu.in. The exam forms are normally sold in the month of February. So keep traking the exam forms through their official website www.jipmer.edu.in
