There is nothing for which you have to be worried in your problem .

For your admission in the Class 12th after completing 11th and having one your gap between this due to medical problem you just need to apply for your admission and submit the necessary the documents like Passing Certificate of Class 11th and the medical certificate etc. .Also you can go for the change o boards for which you have to submit few more documents like Migration , Character Certificate , School Leaving Certificate etc. .
