The online application forms can not be filled up any more as the last date to fill up the online application form was 25-04-2012.
The last date to submit the Hard Copy of the application form along with necessary documents was 30-04-2012.
So, if you have not applied for the test as per the prescribed date, you can not apply for Common Proficiency Test which is going to be held on 17-06-2012.
