Sir, I have mistake in name in my 12th certificate & mark sheet. I have completed my 12th in 2014. But now it is 2019. Kindly give me the procedure to receive a correct 12th certificate and mark sheet.

You are acknowledging the mistake in certificate now but is too late now.

But, candidates with reasonable reason of delay in correction and along with other documents which justify your claim can approach the board for the correction.
