you do not mention your city or state.so please visit the following website and you will get all details.for your help i advice you to fill up thwe form online because it is easier and headche free.after compliting online fillup you will get a 5 page pdf file.on which contains your filled application form,address slip which you have to send at IIT,bank challan copy.and print put challan copy you need to submit nearest SBI bank for deposit fees.

The fees are as follows:
Rs1000/-for Genera/OBC if you are applying offline and Rs500/-for Sc/ST.
Rs800/-general/Obc if you are applying online and Rs400/- for Sc/ST.
Online you can pay through credit or debit card or Netbanking through the payment Gateway of ICICI/HDFC/BILL DESK.
CHALLAN payment is also an option .You can deposit cash in SBI or INDIAN BANK.

You can purchase gate form from canara bank of india ,SBI and zonal offices of gate. Cost of gate form iis 1000/-.
You can also fill up gate form online.

the gate exam form you can get from .
state bank of india , punjab national bank, canara bank,syndicate bank etc....
it depend on the location where you leave .
the fees of the form are 1000/- for general & obc candidates.
for sc / st canidates it is 800/-
you can also collect from post office through post.
