Generally in a form there are questions on your place of birth or you are a resident of which state. If you were born in UP and your permanent address is in Uttar Pradesh then you have to mention Uttar Pradesh as your state. As you are appearing from Bihar for class 12 then it doesnt mean you are from Bihar. In case if there is a column of present address then you have to mention the address of Bihar and for permanent adress write the address of Uttar Pradesh.
