There is no separate application form for the kashmiri migrants .

Earlier the states you mentioned uses to conduct their own entrance test for engineering but now they asnit students to these courses on the basis of JEE main score/rank.

Ofcourse there is a separate counselling programme which is different from the all India joint counselling JOSAA.

You can are required to fill the JEE main exam notification for which is released in December every year.
Making sure you check the kashmiri migrant option during application .

Once results are declared you can go for the centralized counselling or the state whichever is your choice
