Respected Sir/Madam, I would like to know about the essential documents which are required for applying in the Cash Awards for Sportspersons, Haryana. Can you please let me know what are the essential documents which are required? Please tell me asap.

Documents required to apply for Cash Awards for Sportspersons, Haryana are as follows;

Copies of sports achievements,

Certificate which shows for proof of having represented Haryana at the national level,

Age proof,

Domicile/resident certificate of Haryana,

PAN card,

Aadhaar card,

Copy of Bank passbook showing A/c No. and IFSC Code

Download the application form from official website link i.e.

Fill in the application form with required details.

Attach all the required documents with application.

Send the application form along with all the required documents by post to the given below address;

Some of the documents required to apply for the Cash Award for Sports Persons, Haryana are given below:

Proof of achievement in Sports

Aadhaar Card copy

Proof of participation in national level event for Haryana

PAN card copy

Haryana resident proof

Bank pass book with IFSC code and account number

Proof of date of birth

No other documents required to apply for Cash Sports Awards except application proforma.

In general to apply for Cash Sports Awards you should see the eligibility whether you are fulfilling it or not

Then once you are eligible to apply for Cash Sportsman awards then you just fill the application form which will be available in every district og haryana.

Or you can get the form in any youth development office and there you just fill all the required details with accuracy then handover it to district sports office.

But most important thing is you must be citizen of haryana resident.

Then only you are eligible for Cash Sports awards.

- Attested copies of sports achievements

- Certificate/documentary proof of having represented Haryana at the national level

- Date of Birth Certificate from Municipal Committee/Matriculation Certificate/Registrar Births & Deaths

- Haryana domicile/resident certificate

- Copy of Aadhaar card

- Copy of bank passbook’s first page showing A/c No. and IFSC Code
