Documents required to apply for Clinic Plus Scholarship 2019 are mentioned below:-

—> Two passport sized photographs
—> An attested copy of the annual family income of the applicant
—> A copy of the admission proof
—> An attested copy of school fee slip
—> An attested copy of the extra-curricular activity proof
—> An attested copy of the birth certificate/ ration card of the applicant
