Every Government employee is not a gazetted officer .
He is a person who derives it's service and recruitment conditions from gazette of India as per Article 309 of Indian constitution.

Those who fall in class 1 cateogary
Officers of armed forces
Magistrate
Scientists of government funded research organization
Doctors
Engineers
Drug controller
Central and state Government emotes
Patent examiner
Officers of state cadre
District medical officer

Doctors working in the government sector as Medical Officer or General Duty Medical Officer or above designations are class 1 Gazette Officer. All doctors recruited to the various central medical services through the combined medical services exam of the UPSC are appointed to the Group A cadres.
