I want to take 6 subjects for class 12th
from NIOS Board including one optional subject.
But if I fail in an optional subject then
will I be declared as pass or qualify?

You will be declared pass even if you fail in the optional subject in your Class 12 NIOS Board.
You will need to pass in all the rest of the Subjects to be deemed as pass.
It is best for you if you do not concentrate on the pass and fail situation and instead you should concentrate on your preparation for the Board Examination.
You should concentrate more on the Subjects which you have taken so that you can eventually score more marks in your Class 12 Board Examination.
This is the best option for you and you should be rest assured that you will be deemed pass if you are able to pass in all the 6 subjects and even if you fail in the optional subject.
This is what the rule of the NIOS Board suggests.

In 12th NIOS, if any candidate is pass in their 5 main subjects then he/ she will be considered pass irrespective of the fact that he/ she fails in 6th /optional subjects.

So, do not worry about optional subject, just focus on the main subject to secure better marks .

Concentrate only on the main subject, read according as syllabus.
Do not try to study anything unnecessarily which is out of your syllabus as it will affect your marks in finals.

Since, you are allowed to chose the 5 subjects of your choice in your 12th Grade, but if you chose to 6 subjects and unfortunately you failed in the optional, then yes you will be treated as pass but you won't be treated as qualified. So, better not to take risk in these field and prepare for the exam very well, so that you get passes as well as qualified also.

Yes, you will be qualify for 12th after fail in optional subject in 11th class.
According to rules of NIOS you have to just qualify in 5 main subject in 11th class to get admission in 12th class in NIOS.
So don't worry only focus on your study and get good marks in 12th board.

If some one is fail in two main subjects and pass in three subject and that 3subject include 1main subject then he is pass or not
