~> At the home page click on - STUDENTS

~> In this section click on - Admissions - 2015

~> Under this section, click on – Forms

~> At this page you will find many forms like Ph.D, M.Phil., PostGraduate, Previous Under Graduate, Certificate/Diploma/Others.

~> After that you can selection the option of PG Form ( M.Phil )

~> At last you can download the PG admission form.
