Hello I gave my mhtcet exam in last year, in this year i gave cet examination Against , so which options to choose in resistration process. 1) new resistration 2) already registered.

You need to choose new registration since you will be registering for the first time for the upcoming exam.
Already registered, option is for the candidates who have just done the initial registration and got the registration number and password and yet to fill the complete form.
That's why there are 2 options :
New registration for the first time user
and Already registered for those who have got registration number and password but yet to fill and submit the complete form.
