After visiting this site, register yourself with relevant and authentic information.

Now, login yourself and fill your personal details.

Then, take the printout of the filled form.

Attach this printout with the necessary documents required with this forum and hence you are done.

Submit all the documents clubbed together to your College at the respective department.
