where will I get the application form in kolkata for mbbs 2011 in DUMET.

I put up at sagarpur, so which is the near centre to get my application form.

Dear Friend

There are two ways of purchasing DUMET application form and information brochure.

1. By HAND: To get the application by hand one is required to make a Demand Draft/
Banker's cheque of Rs.750(Rs.550 for SC/ST category) and visit the DU office.
The address from where application form can be obtained is
The office of the Deputy Registrar,
Faculty of Medical Sciences,
6th Floor, V. P. Chest Institute Building,
University of Delhi, Delhi-110007
The demand draft/banker's cheque should be made in favour of
‘Registrar, University of Delhi’ payable at Delhi.

2. By POST:
If you are staying outside Delhi then you can get it by post.
Rs.50 will be charge additionally.
Send the DD/Banker's Cheque along with a self addressed envelop
of 12″ X 10″ upto 11th February, 2011

SC/ST/PH candidates are required to bring(If getting application form by Hand)
or send a copy of the Caste Certificate/PH Certificate (If getting application form by post)
for obtaining Bulletin of Information.

DUMET form can be obtained in hand from the Lady Hardinge medical college, Maulana Azad medical college and university college of medical sciences only.

Since you are in kolkata and it is not feasible for you to go to delhi and procure the same, so i suggest that you visit the following link here you may obtain the form easily.
The form is ONLINE, so no trouble at all:

You are required to submit the duly filled and complete form in all respects with requisite Demand Draft, to the office of the Deputy Registrar, Faculty of Medical Sciences, 6th Floor, V. P. Chest Institute Building, University of Delhi, Delhi-110007 on or before 08th March,2011 upto 03:00 P.M.

Also, you can obtain the form via post for which you are required to pay Rs. 800/- (Rupees Eight Hundred only) for General and OBC category candidates and Rs. 600/- (Rupees Six Hundred only) for SC/ST/PH category candidates, along with a self addressed envelop of 12″ X 10″ upto 22nd February, 2011.

DUMET-2011 (Delhi University Medical Entrance Test-2011) for MBBS/BDS courses: 15th May, 2011, Sunday

(i) Sale of Bulletin of Information w.e.f. 24th January, 2011.

(ii) BY HAND: for ` 750/- (` Seven Hundred Fifty only) for General and OBC category candidates and `550/- (` Five Hundred Fifty only) for SC/ST/PH category candidates between 09:30 A.M. to 03:00 P.M. on all working days from 24th January, 2011 to 07th March, 2011. No cash payment will be accepted.

(iii) BY POST: for ` 800/- (` Eight Hundred only) for General and OBC category candidates and ` 600/- (` Six Hundred only) for SC/ST/PH category candidates, along with a self addressed envelop of 12" X 10" upto 11th February 2011.

Applications Form may also be downloaded from our websites www.fmsc.ac.in and submit the same, duly filled in and complete in all respects with requisite Demand Draft, to the office of the Deputy Registrar, Faculty of Medical Sciences, 6th Floor, V. P. Chest Institute Building, University of Delhi, Delhi- 110007 on or before 07th March, 2011 upto 03:00 P.M.
Note:

1. The last date for receipt of request for supply of the application form alongwith BOI by POST is 11th February, 2011.

2. The candidate should mention his/ her name, address and telephone number at the back of the Bank Draft/ Banker’s Cheque. The candidates are advised to retain a photocopy of the Bank Draft/ Banker’s Cheque.

3. Incomplete applications will be summarily rejected.

sorry dude but you can not get the application form in Kolkata.
They are available only in delhi.
You have only two ways either you goto DELHI..
Or as i Suggest .. for outstation candidates you can get the application form by post.
The cost of application is 88 rupees for general & obc candidates
and 600 for SC/ST
Forms are available since 24th jan 2011.
Send the DD ..( you have to make 50 rupees extra payment for Postal charges so include that too)...along with a self addressed envelope upto 11th feb,2011.
