Apart from obtaining the the application forms from the banks, you may also apply online through the official website of IIT's.

You can fill the IITJEE forms online as well as offline.To fill the online form the form are available from 31.10.2011.The offline forms are also available from 11 nov from banks.The list of the banks are available in the website of the IITJEE.
