The application forms for the All India Pre Medical Test has not been yet made available anywhere but as soon as the notification is issued, you may obtain the application form from your nearest branch of Canara Bank.
Apart from purchasing the form from the Canara bank, you may also apply online through the official website of AIPMT.
For up to date information, you must refer to the official website of AIPMT.
