Hello, I need to know that what are all the documents required for applying in UP Combined Pre Ayush Test of 2019? I want to apply in this entrance exam but I came to know that there are some documents which I have to submit. Please tell me about those documents. Thank you!

Documents Required to Apply for UP CPAT examination:

>> A valid email id

>> A valid contact number which should be active during the online registration as all important information and OTP will be sent to this number.

>> A scanned copy of a recent passport size photo in JPEG format.

>> A scanned copy of your signature.

>> A scanned copy of your thumb impression.

>> Aadhar Card / Voter Card / Pan Card

>>Caste certificate (if any)

Documents required to apply for UP Combined Pre Ayush Test 2019 are as follows:-
12th class passing mark sheet.
Domicile certificate (compulsory).
ID Proof and passport size photos.
Caste certificate (compulsory for reserved candidates).
Character certificate.

You need to have the following documents to apply for UP Combined Pre Ayush Test

- Domicile certificate (mandatory).

- ID proof and passport size photos.

- Caste certificate (For claiming reservation quota).

The documents needed for the UP CPAT 2019:

- Candidates Birth Certificate, Address proof with permanent person of UP.
- 12th Marks sheet of the candidates and passport size photos.
- Candidates must fill the application form in online mode and fill the details as per the original Documents.

1. Personal valid Email id where you will get email.

2. Mobile Number where you will recieve OTP during
registration.

3. Scanned copy of your signature.

4. Scanned copy of your thumb impression.

5. Scanned copy of your passport size photo in JPEG
format.

6. Aadhar Card / Votar Card / Pan Card

7. Domicile Certificate.

8. Caste Certificate for claiming reservation quota.
