I have filled online application form for MBBS. I have not received the REGISTRATION SLIP / ACKNOWLEDGEMENT SLIP for the same till now. I have created the account and also have paid the amount 1025/- from SBI Main Branch Aligarh (Branch code no.- 604). The account no. - 31629888649 ;challan no. - 1051029459 ; jounal no.- 015195008 on 8 march. please send me the REGISTRATION SLIP on my e-mail address [email protected] as soon as possible.

If you have any received the registration slip for MBBS. You can submit a request for duplicate registration slip. You must have the documents mentioned below:

1)An affidavit of Rs.10/- non-judicial paper duly attested by the 1st class Magistrate.
2)Attested photocopy of date of birth certificate.
3)Two passport size photographs.
4)Two adhesive signature slip
5)Handwritten application.
6)Registration fee i.e. a DD for Rs.2000/- in favour of Secretary, Medical Council of India, payable at New Delhi.
7)Attested photocopy of permanent registration certificate.
8)F.I.R. in original.
