Yes, IIT Forms are available in Punjab National Bank but only in the IIT Roorkee Zone which comprises of :-

Availability of IIT JEE Forms

The IIT JEE Forms are available only at select outlets of Union Bank of India or at your nearest book depots..

The last date for submission of the filled up forms for IIT JEE is 15th December,2011 so you should better hurry up..

the exam will be conducted on 8th April,2012..

The rate for the IIT JEE form is as follows :-

For General category male : Rs 1800
For OBC/SC/ST males : Rs 1000
For all Females : Rs 200

In order to appear for the IIT JEE 2012 you must fulfill the following criterion :

1) You should be a 12th pass out or 12th appearing with an aggregate percentage of 60% or above in Physics,Chemistry and Maths

2) You should have passed out your 12th in 2011 or appearing in 2012..

If you fulfill both the above two conditions..only then you shall be eligible for appearing in the IIT JEE 2012..
