From which place we can recieve jelet form. Can wf recieve it from siliguri?

The application form for JELET 2012 can be obtained from the following branch of Axis Bank in Siliguri:

Siliguri-734003

The Joint Entrance Exam for Lateral Entry (JELET) conducted by West Bengal Joint Entrance Examination Board will be held on 24-06-2012.
The application forms can be obtained till 05.05.2012.
The last date of submission of the application form is 10.05.2012.
