Model Hospital ESIC-Kollam, when new employment application forms issue

A new employment application form for Mallesham Education Society, Kollam (Kerala) affiliated Edusat Model Hospital SEI is being drawn up. The form will be categorized into two parts - Instructions and Common Questions.

While the questions are common to all of SEI hospitals under it, a surgeon's statement will be required as per government policy ex-cathedra. Technically-minded applicants can go on filling the forms in English, Malayalam and Hindi online at *www.edusatformularexamplingapp.org ; however they need to call up the centre helpline first on 9889930982 or 9889990345 to get a customised form immediately.

These are expected to come out by November 10 or early December on behalf of Edusat Model Hospital Kollam solely through customization *of the format of instructions alone owing to the fact that it has its only single-handed repository that is Malayalam language-bound
