If you are willing to apply for the upcoming Common Proficiency Test, first of all, you will need to register yourself and in order to do so, Click Here.

Once you complete the Sign-up process and have got the username and password, you will then need to Log-in and fill the online application form.
The last date to submit the application form (online) is 25-04-2012.
