The notification for the admission process will be published in the leading newspapers during April to June. Interested candidates can refer to these notifications and follow the instructions. The application forms will be available for purchase in designated test centers. Applicants can also visit the official website of the concerned institution and download a copy of the application form.

You have not mentioned your state so it is not possible for me to tell you exactly where to get the form.
In fact even in a state there are atleast a dozen centres where you can get the form.
You can also get the forms online.
