The Application Kit, for both offline and online mode of application are available at selected branches of Axis bank in the following States:
  • Maharashtra
  • New Delhi
  • Goa
  • Gujrat
  • Madhya Pradesh
  • Uttar Pradesh
  • West Bengal
Unfortunately, the list do not include the State of Bihar.
