What should be done in situation when ICAI issuing letter saying INCOMPLETE FORM?


Sir, I applied for CPT exam on 29/06/2017. But have not received any registration letter till now. Got SRO number, study material, hall ticket for exam which is on 16/06/2019. But yesterday I got a letter from ICAI Joint Secretary (exams) regarding incomplete filling of form. What should I do now?
