The Entrance Examinations for admission to the MBBS program is offered by Institutes, Directorate of Medical Education of each state, Central Board of Secondary Examination, Armed Force Medical College, All India Institute of Medical Science and various other regulating bodies.
In order to get the application form for some of the most reputed entrance examination, refer to the official website of the following:

All India Pre-Medical Test- Conducted by CBSE
