While mentioning "application form of Common Proficiency Test", it may refer to either the registration form required to submit during registering for the course or it may refer to the examination form which is to be submitted to take the test.
Now, since you have not mentioned it specifically and assuming that you have already registered for the course and willing to submit the examination form,

Sir, I am registered in ICAI with my delhi address but now i've shifted in vaishali ( ghaziabad) and had no links with my delhi address. Now at the time of filling the exam for Dec. 2016 I want to use my new address of Ghaziabad and I also had a choice of a different address in Delhi. I just want to know that can I use a different address in exam forum other then my registered address so I can get my result delivered on my new address?

You have to send to the address from where you have got letter of CPT registration Number.
For address just check upper portion of Letter.

I live in Uttar pradesh but I will be living in Delhi in future. Can I send my form to Delhi regional office?
