The application form of NATA 2012 shall be sold in the designated bank branches of
AXIS bank.

The complete list of bank branches of AXIS bank for sale of NATA application/registration
form is given in the NATA 2012 Information Brochure which is attached here with this
message.
Refer the attachment.

The filled in application form has to be submitted at the Test Center where you want
to appear NATA.
Along with your registration form you have to submit attested xerox copies of
10th board mark sheet, one photo ID proof and recent passport size photographs.
