Well dear you haven't mentioned for which MCA entrance exam you are asking for application.

However, I am discussion two most famous ones:-

AIMCET: Candidate must have passed +2 in any stream with mathematics as a main subject. Candidate should also be a graduate with at least 3 year degree. Final year student can also apply for this entrance. From year 2005 admissions to MCA programme at IIT Roorkee are also made through JAM which were previously only through AIMCET.

IITJAM Important Dates

Issue of application forms and information brochures at Punjab National Bank and Online Registration: 22 December 2010

Last date for issue of application forms and information brochures at Bank: 21 January 2011.

Last date for Online Registration on the website: 24 January 2011.

Date of JAM 2011 Test: 08 May 2011 (Sunday)

NIMCET:- Apart from having MAthematics as a main subject in +2 candidate must have also obtained there bachelor such that they must have opted for anyone of the following as there MAin subject in Bachelor dergee and also have scored at least 60% aggregate.

Mathematics,Statistics,Business Mathematics,Discrete Mathematics,Mathematical Foundations of Computer Science,Business Statistics,Computational Mathematics, Elementary Mathematics,Elementary Statistics.

Application form of which MCA entrance exam do you want??

because there are many entrance exams taking place all over India for MCA....

1) Maharashtra MAH MCA-CET -- it is the MCA Common Entrance Test in Maharashtra.

the exam date of MAH MCA-CET is 6th March 2011.

But the last date for receipt of form is over which was 18 February 2011..

the application forms were available at some selected post offices.

2) NIMCET -- it is the Entrance Exam for MCA in National Institutes of Technology (NITs).

the exam date of NIMCET is May 15, 2011.

last date of online application is March 28, 2011.

last date of offline application is April 11, 2011.

3) AIMCET -- it is the Entrance Exam for MCA in IITs.

First fall you should not mention your state ,where you belongs because different collages have different test .
only one thing to be done that you should search on net by typing MCA exam.

As you didn't mentioned for which MCA exam you want to apply for. kindly mention your full information regarding the MCA exam. Anyways you must have mathematics as a main subject in 12th or equivalent.
