Is there any concession given for visually challenged passengers in train travel?


Is there any concession given for visually challenged passengers in train travel? I have 40% vision problem. So, can you let me know if am eligible for getting the concession? What is the procedure followed for the same? It will be helpful if you can answer me soon.
