i am gpat qualified student and studying in m.pharm 2nd year. please give me gpat scholarship form.

You need to go through certain procedures to obtain the scholarship.
Procedure for Obtaining
Fill the GPAT form attached below and deposit it at your College/Institute administrative staff.You even need to attach the xerox copy of GPAT Score card along with it as you said that you have qualified for the examination.The college authorities will send the AICTE Approval letter to the GPAT authorities in the address mentioned below
They will send forward this form to AICTE office, New Delhi at following address

ALL INDIA COUNCIL FOR TECHNICAL EDUCATION(AICTE)
7th Floor, Chanderlok Building
Janpath, New Delhi-110 001

At present stipend of GPAT scholarship is approximately around Rs. 7500-8000/pm.Please make changes in the application form attached below i.e the year needs to be changed.

GPAT- GRADUATE PHARMACY APTITUDE TEST scholarship can be obtained by filling the Form for Scholarship attached with this file bellow and the same will be deposited with your institution where studying with an attested copy of GPAT score card. The authority of the institution will send the submitted documents to AICTE office Delhi with the approval letter of AICTE to the Institution where you are studying.

Is there any effect on job or admision in p.hd. after m.phrm from the colege which are not approved by AICTE.

I am M.Pharm 2nd year student can i get benifit from GPAT if i am not doing Phd.
