To apply online, for the common proficiency test, one has to go to www.icaiexam.icai.org.
Form this official website of ICAI, candidates may apply for exams online.
As you have asked about the common proficiency test which is to be conducted on 18-12-2011, the last date to apply for the same was 28-10-2011.
