The basic format is that you mention your Name in bold, below that mention your address. Your resume should have an objective of what you would like to achieve in like. Then your work experience if any. After that your education qualification. Finally you can give details like achievements, personal details etc.

I have attached a format of it. Go through it. Generally this is the one mostly used and satisfies all the requirements.
