Which form i have to fill-up for dob correction of madhyamik admit card? I am much worried for the Correction Of Madhyamik Admit Card

To correct your DOB(Date of Birth) in your Madhyamik Admit card, you have to download the following application form, which is attached below and you should fill up it and you have to submit that application form at any of the Regional offices, which are specified in the application form(attachment).
