As your question suggest you may have lost your registration number, and if is it so then you must contact your nearest regional office. As you have mentioned that you are from Madhya Pradesh, the you must contact:

I have applied for CPT 2012, on 26th march, 2012, and I have not given any Registration No. so far, and presently the Regn. No. is being asked to fill up in the application form for CPT, and it is also stated in the instructions that if any left blank in the form the form is liable for rejection. As such I may kindly be advised to know the CPT- Registration No.

cpt number is given when u have cleared cpt and its valid for 6 attempts and it normally start from SRO,WRO,NRO depending upon region pf students and it is given after approx 45 days of clearing thr cpt exam

You can applying for CPT Exam through Online Portal i.e. http://icaiexam.icai.org
You might have entered your personal email address and mobile number.
Then after the payment of fees after submission of application form.
Complete details will be send to your email address.
After that Registration Number can be retrieved from your email address and also, you can get it through the SMS received on your mobile phone.
