There is no provision for online application for the MBBS Entrance Exam conducted by Banaras Hindu University.
The application forms has to be obtained offline from specific branches of Bank of Baroda.
The application forms has to be sent by registered post at the following address:
The Director (PMT CELL)
Institute of Medical Sciences
Banaras Hindu University
Varanasi-221 005

The last date to submit the application form at the address mentioned above is 15.03.2012.
