Hi friend
Notices for availability of application form for medical colleges are usually advertised in major newspapers and websites of the respective bodies which conduct the exam. the cost of the form varies but usually is in the range of Rs. 800-1500. Make sure you keep checking the websites for notifications and also speak to seniors to know exact procedures
