Although you are registered in Punjab and want to operate your clinic in Delhi, this comes under legal offence, and any one can sue you for this if they came to know about this.

This is similar to, driving a vehicle in Delhi which is register to the RTO of Punjab. So, Delhi police will for sure take an action over Punjab vehicle.
