Please let me know from where can i get ENTRANCE EXAMINATION FORM OF AIPMT .

You can download the application form through the official website of AIPMT. The application forms will be available from the second week of November 2012 to the last week of December 2012. You should make the online payment either by credit card or by sending a DD along with the printed form. The candidates from open category should pay a fee of Rs. 700/- and those from reservation category has to pay Rs. 300/-. To apply for the post, the aspirants must have an age between 17 and 25 years. The candidates must complete 12th from a recognized board with English, Biology, Chemistry, and Physics.

Online application is the only mode of application by which you can apply for the All India Pre Medical/Dental Test-AIPMT.
The online application form is available on the official website of AIPMT.
The last date of submission of application form is:
25.01.2012- without late fee
24.02.2012- with a late fee of Rs.1000

The examination will be conducted on the following dates:
Preliminary Exam- 01-04-2012
Final Exam- 13-05-2012
