How can I apply in State Scholarship Scheme if secured 98% in Intermediate of UP Board?

Definitely you will get the state scholarship after scoring 98% in your intermediate from Uttar Pradesh board. For getting the scholarship you simply have to visit at the official website of the UP board. You can go to official site by just clicking on the below given link.
http://scholarship.up.nic.in/index.aspx
After that you have to click on the Student and then click on new registration. Then tap on "post metric". After that you have to tap on the fresh registration. After that you simply have to complete your form and submit.

You can visit Samaj Kalyan Vibhag and take application form from there and apply for scholarship with full details and income certificate of your parents.
