I m doing my 10+2 from a private.i would like to know that what has to be fille in certain columns of the form like:
1.School Board of class XII(column-14)
2.Type of institution of class XII(column-15)
3.Name of the qualification passed/appearing.
4.Board/university
5.Name and address of the school appearing.
Please Guide me

Well I am telling you that what to fill in the desired coloumn.
1.- CBSE, ICSE or OTHER STATE BOARD
2.-PRIVATE or GOVERMENTAL
3.-SSC or Class 10th
4.-Either CBSE or ICSE or other UNIVERSITY NAME
5.-Either PRIVATE CANDIDATE or The SCHOOL NAME from where you are filling form.

1. the name of the board which conducts the 12 th exam....
2. name of the institute ....
3. if u are in 12 th then write appearing 12th....and if u have passed 12 then write padded 12th.......
4. it can be CBSE,ICSE,other state level boards......
5. write the name of the institute and the address where it is located

1.in this part you have to fill the name of you board,from which you studied.For example-cbse,isc,wbchse,or any other state board.

2.in this column you have to fill that your school is goverment or private once's.

3.in this column you have to write the name of your 10th exam (for example-icse etc) and the name of the class 12 exam you are appearing (for example-isc etc).

4.here you have to mention the name of your board (weather cbse or any else).

5.here you have to write the name of the school through which appearing or giving exam from private.

if you are filling the 12th exams form then all the necessary information about filling are there in apllication form , please read the application form fully.

i want to suggest to read the instruction in the application carefully then fill it,you will get every information from there.
for you 1st question-you should write name of school boad like cbsc or state boad,icsc
for question 2-private or government
for question 3-10th or ssc
for question 4-same as question 1,
for question 5-write name and address of your school.

I have filled the CBSE private form for 2011 for ADDITIONAL SUBJECT and obtained my form from IIS School,Jaipur. In jaipur only IIS/KV1 provide form for private candidate for 10 Rs.
NOTE: (1) In JAIPUR Syndicate bank does not provide CBSE private form 2011. Only IIS/KV1 provide in JAIPUR city.
(2) For other places,you should first try to SYNDICATE BANK of your place. If you don't get from there then go to any CBSE school of your place or contact to DELHI office and ask for the same.

REMEMBER:: LAST DATE FOR SUBMISSION/RECEIPT OF FORM IN REGIONAL OFFICE AJMER IS [15 SEPTEMBER 2010(on or before 1pm)],WITH late fees (30th October 2010).

i had sent two letters to get private application forms yet i have not received it and last date with late fee is 30 oct wat can i do pata nhi kyun nhi bhej rahe ye chennai regional office waale form ko zindagi barbaad kar rahe hai

Your question is still incomplete please mention your board- cbse,icse or state . But in common all the form starts registering from the december month and you receive your admit card before exam.
