1. BIRTH CERTIFICATE (ORIGINAL & unattested XEROX COPY)

3. BOTH PARENTS ARE TO BE PRESENT. Child need not come.
4. A STAMP SIZE PHOTOGRAPH OF THE CHILD with name written
