How to take admission in 12th after completing 11th science 6 years back


i have complited 11th science 6yrs back .i want to take an admission to 12th .can iget &what would be the procedure of practicals ?

i have completed my 11th class through science stream and after that i didn't given exam of 12th. now i want to complete my 12th through the same stream.
there has been 15 year has been passed
