The AIEEE forms are available in SBI. Since you belong to varanasi, the forms will be available in all the branches of SBI in Varanasi.
Some of them are
1. SBI in Nadesar
2. SBI in Visheshwarganj
3. SBI in Assi, etc

You can get the Application form with the Information Bulletin by post from the CBSE or personally from Regional Offices of CBSE, designated Institutions and the designated Branches of Syndicate Bank.
