Your Form Got rejected because you haven't filled the Form correctly.

So just Follow these steps

Steps to check Application status:-

1) Go to CPMT site
2) Click application status
3) Enter your NUmber.
4) You will Get the response

It will show you the Reason why it has been rejected.

pls tell i do mistake in ma photo ..i compose date of photo and name via computer so its will be rejected or accepted
