Will there be any problem, if my headmistress put her signature in CPT application form in the column of name?


In my CPT exam form the headmistress put signature instead of writing her name in the column of name of the head of educational institution. Is there any offense?

No. If the signature is visible enough to recognise the name it can work. However if it is not. Than I would suggest you to fill another new form of CPT (Common Proficiency Test).
